TESTIMONIAL RELEASE AND PERMISSION: Speaker Only

The undersigned, (“Speaker”), (“Speaker”), grants to QIAGEN, LLC (including its affiliates, agents, employees, representatives and licensees, “Company”) the absolute right and permission to copyright, make, reproduce, publish, republish, exhibit, distribute, record, display, edit, print, broadcast and otherwise use Speaker’s testimonial about
(“Testimonial”) together with Speaker’s name and job title, exclusive of Speaker’s employer’s name (collectively, “Testimonial Information”).
Speaker also consents in perpetuity to Company's use of the Testimonial Information, or any portion thereof, in any advertisement, marketing materials or other media (collectively, “Advertisements”) of Company.
Speaker acknowledges that Company will own all intellectual property rights in the Testimonial and any Advertisements and that Speaker will not receive any compensation for granting the rights and releases given in this Release and Permission.
Speaker waives any right s/he may have to inspect or approve any Advertisements or any derivative works thereof or any media displaying any portion of the Testimonial Information or Advertisements. Speaker also waives and releases Company from any claims or causes of action that s/he may have related to the Testimonial or Advertisements including, by way of example only and without limitation, any right of publicity or privacy claims.
Speaker has acknowledged the authorization, release and agreement set out above prior to execution of this Release and Permission and is fully familiar with its contents. This Release and Permission will be binding upon Speaker and Speaker’s heirs, legal representatives, successors and any permitted assigns.
